Advisory Non-Retirement Account Application

LPL Advisory non-retirement accounts use this form for adding, removing account holders, or registration or account type changes.

Advisory Retirement Account Application

For existing accounts, this form should only be used for adding or removing account holders, or to change the registration or account type.

Brokerage Non-Retirement Account Application

To be used for all brokerage, direct business and TAMP/TPIA non-retirement accounts. For existing accounts, this form should only be used for adding or removing account holders, or to change the registration or account type.

Brokerage Retirement Account Application

To be used for all brokerage, direct business and TAMP/TPIA retirement accounts. For existing accounts, this form should only be used for adding or removing account holders, or to change the registration or account type.

Use this form to have LPL Financial (“LPL”) credit and/or debit your bank checking or savings account through the Automated Clearing House (ACH) system.

Retirement ACH Distribution Form

Use this form to request an ACH or Address of Record Check or in-kind distribution from an LPL Financial individual retirement account or an LPL Coverdell Education Savings Account.
